Nursing Homes Support Service Aims

• To provide specialist clinical support in the care of elderly people, to staff and owners of care homes with nursing.


• To provide a link to other multi-disciplinary healthcare services in the PCT for the staff to support their care of elderly people.


• To support GPs with their input into nursing homes
  in regard to advanced care planning, creation of clinical care pathways and prescribing.


• To enable elderly people to remain in their home environment and prevent unnecessary admissions into acute care settings.
